Responsibilities:

Evaluates patients referred to the Organization; identifies insurance coverage, negotiates payment or per diem rates; plans and coordinates referrals for home health services.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Facilitates intake process by procuring patient information, including but not limited to demographics, face-to-face encounter document, clinical data, prescribed home health or hospice services, insurance assignment of home infusion benefits, and following all patient acceptance criteria policies and procedures.

Collaborates with the facility’s discharge planning personnel to facilitate safe discharge to home health or hospice.

Receives and processes referral information for potential patients who require home health or hospice services.

Demonstrates ability to manage incoming information, including but not limited to portals, faxes, and email and enter the information accurately and timely into the EMR.

Participates in agency stand-up and stand-down meetings.

Escalates potential delays in clinical care due to incomplete referral information to the Intake Supervisor and Executive Director/Director of Patient Care Services.

Establishes and maintains positive and productive working relationships with referral sources and internal customers.

When needed, coordinates the discharge plan with the referral source/discharge planner.

Maintains comprehensive working knowledge of organization’s contractual payer relationships.

Adheres to all company policies and procedures.

Other responsibilities as assigned.

Qualifications:

Minimum of one (1) year of experience working in home health care, hospice, or managed care preferred.

Computer skills, including knowledge of relevant software.

Ability to establish and maintain effective working relationships.

Must have effective multitasking, communication, organizational, and interpersonal skills.
