Physical Therapist Home Health - Weekender
At Visiting Nurse Association of Ohio, we are looking for dependable, compassionate people interested in gaining experience and becoming an invaluable resource to our organization. VNA of Ohio is now hiring a Physical Therapist for Weekends.
Working Saturdays and Sundays, The Physical Therapist (PT, DPT) in Home Health – Weekender, is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.

Responsibilities:
Essential Functions:
Performs initial assessment or admission to agency and determines patient treatment needs
Develops a plan of care with measurable goals and specific discipline related treatments in conjunction with physical referral/orders
Provides direct physical therapy services as outlined in the plan of care including performance of physical therapy procedures, instruction of patient and/or caregiver, recommendations for equipment or adaptation of home environment
Re-evaluates and revises plan of care as needed with physician approval
Facilitates total care services to patients by making appropriate referrals to other disciplines by obtaining physicians’ orders or by consulting with case manager
Submits all documents in a timely manner
Performs other related duties as assigned
Qualifications:
Qualifications and Requirements:
Bachelor of Science Degree in Physical Therapy required, post-graduate degree preferred
One year experience in a supervised healthcare setting
Licensed, in good standing, to practice physical therapy in the State of Ohio
CPR certification required
Valid Driver’s License
