Full-Time Evenings 2pm-12:30am

Responsible for all aspects of Pharmaceutical care provided at the Hospital: Evaluates medication orders at verification to promote rational cost effective drug therapy, Supervises accurate dispensing of product to positively impact patient outcome,   Supervises technical staff to assure safe and accurate completion of Unit Dose and IV admixture services, Supervises medication storage areas both within and outside pharmacy to assure drug safety and security, Provides patient education about new drugs and potential adverse reactions to drugs as requested, Assists with Medication reconciliation activities, Applies all Pharmacy related laws and required documentation, and Documents interventions in appropriate system.  Also provides Renal dosing program, kinetic & dosing service, anticoagulation monitoring and dosing service as well as assistance precepting Pharmacy Residents as requested/scheduled.  Provides these services to neo-natal, pediatric, adolescent, adult, and geriatric customers.

Responsibilities:

Patient Care Distributive Duties

Verifies all orders generated on shift, keeps work flow up to date and current, and cleans up old orders as soon as possible.

Completes verbal hand off communication to next shift, to ensure continuity of quality patient care and follow through in interventions.

Screens and affirms appropriateness of orders including allergy check, formulary compliance, pregnancy & lactation status, dosing, duplicate orders etc.

Documents adverse drugs reactions & medication errors in the RL system as they occur throughout the medical center.

Fields and investigates drug information questions. Uses resources appropriately and colleagues as needed to ensure timely and accurate responses.  Documents as necessary.

Checks and assures high quality of IV Room products every time. Ensures timeliness of delivery of final product, utilizing means of maximum efficiency.

Enters TPN orders into compounder computer system accurately and timely.

Maintains good rapport with peers, nursing staff, physicians and other departments.

Becomes involved with Performance Improvement Projects, Clinical Initiatives, and other work projects as assigned, and completes them in a timely manner with attention to outcome. Uses data, good communication tools, and multidisciplinary approach as strategy.

Completes Pharmacy closing activities at end of shift.

Patient Care Clinical/Additional Duties

Completes anticoagulation monitoring and kinetics monitoring demonstrating high skill level

Assists with training: Students, Technicians, Residents, and new pharmacist’s orientation. Coaches and instructs in proper procedures, focusing on quality outcomes.

Identifies issues and optimizes care using the provided clinical platform Bugs/Drugs, Renal dosing, duplicates, and other clinical rules that are broken.

Documents clinical work using EPIC IVents.

Willingly takes on other clinical tasks and initiatives as assigned.

Completes at least 2 hours of stroke education CE annually.

Basic UPH Performance Criteria         

Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.

Demonstrates ability to meet business needs of department with regular, reliable attendance.

Employee maintains current licenses and/or certifications required for the position.

Practices and reflects knowledge of HIPAA, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.

Completes all annual education and competency requirements within the calendar year.

Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.  Takes appropriate action on concerns reported by department staff related to compliance.

Education:



Graduate of accredited College of Pharmacy

Pharm.D., Residency

Experience:



Prefer one (1) year of hospital clinical experience, Residency

License(s)/Certification(s):



Current licensure in good standing to practice as a Registered Pharmacist in Iowa

Valid driver’s license when driving any vehicle for work-related reasons.

Completes at least 2 hours of stroke education CE annually.

Board Certification Preferred

ACLS/PALS Preferred

Knowledge/Skills/Abilities:



Writes, reads, comprehends and speaks fluent English.

Multicultural sensitivity.

Preparation of IV Sterile Products

Customer/patient focused.

Critical thinking skills using independent judgment in making decisions.

Other:



Use of usual and customary equipment used to perform essential functions of the position.
