The Social Worker is responsible for coordinating and implementing post-discharge plans in coordination with the Case Managers through the use of Electronic Medical Record (EMR) and Allscripts. They are also responsible for assisting with advocacy and referrals to other community resources

Qualifications:

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

Bachelor degree required; preferrably in Social Work

3 years' experience in case management, discharge planning, social work, managed care, or

professional counseling, required.

Experience in a hospital or healthcare setting, preferred.

LCSW Preferred

BLS 

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
