Ho'okele is looking for an individual to join our team of RN Navigators.
As an RN Navigator, you will have the important role of assisting seniors and their families to coordinate services that will enable them to remain at home safely. You will also serve as an advocate for them and act as a liaison between the senior, their family, and the healthcare providers. The main objective of the RN Navigator is to enable individuals to effectively engage in self-management of their health and well-being
Key Responsibilities and Accountabilities include:
Ensuring continuity of care for seniors by coordinating necessary services
Improving satisfaction with healthcare experiences by providing personalized support and guidance
Delivering evidence-based and cost-effective care to seniors
Act as a family liaison and provide emotional support to both seniors and their families
The specific tasks will vary depending on the unique needs and preferences of each individual.

Requirements
To be considered for the position of RN Navigator at Ho'okele Home Care, you should meet the following requirements:
Bachelor's degree in nursing
Active State of Hawaii RN license
Minimum of two years of experience in clinical nursing
Current and valid State of Hawaii driver's license
