Overview:

The Specialist, Chronic Disease Prevention will oversee Evidence-Based Health Interventions (EBHI) program operations for YMCA of the Pines, Gloucester County YMCA and support the the vision and goals of the Y’s focus on health seekers.

This role is a year-round part-time position budgeted for 12-15 hours per week, with a 3-month paid training period upon enrollment.   

Responsibilities:

External Activities: 

Direct participant recruitment and engagement activities including development and maintenance of relationships with the health care community and other partners who can refer participants and/or provide program locations in partnership with EBHI Champion. 

Identify program sites that meet the needs of the target population. 

Manage master schedule for program(s). 

Encourage a high level of awareness and build buy-in for chronic disease prevention work among all branch staff, senior leaders, members, and the public. 

Respond to inquiries about the program from the public and health care community. 

Follow up with referring health care providers and payor partners if applicable. 

Facilitate communication of participant progress to referring health care providers. 

Maintain existing partnerships and develop new partnerships with the health care community and other organizations serving people affected with chronic disease. 

Support EBHI Advocate by planning for growth and long-term funding, staffing, partnership, and resource sustainability. 

Serve as point of contact and program representative for association. Provide implementation updates to Y-USA, as requested. 

Participate in Y-USA technical support activities pertaining to EBHIs. 

Internal Activities: 

Engage senior leadership at the corporate and branch levels to garner support and to communicate program successes and challenges. 

Coordinate and communicate with Y staff across all association program sites/branches. 

Ensure association facilities are welcoming, knowledgeable, empathetic places of support for health seekers and people living with chronic disease. 

Recruit, retain, and supervise the staff delivering EBHIs. 

Support skills and well-being of EBHI program facilitators. 

Organize training and ensure certifications are maintained for all EBHI facilitators. 

Engage marketing staff to ensure adequate publicity for and marketing of the program. 

Obtain equipment for use by program staff to support successful program delivery. 

Manage participant data in athenaNet, REDCap, and/or ODES to assess quality of data entry and participant progress and conduct quality assurance observations. 

Qualifications:

Experience implementing evidence-based program models, including targeting specific or underserved populations, managing referral relationships with health care providers, and implementing quality assurance and data collection protocols. 

Strong attention to detail and commitment to program fidelity. 

Ability to build strong relationships, inspire confidence, and work effectively with a wide variety of stakeholders (health care providers, public health officials, payors, health seekers, and senior Y leaders and front-line staff). 

Experience supervising front line program staff, and ability to model skills necessary for their roles. 

Understanding of the wants, needs, and interests of health seekers working to prevent or manage chronic diseases. 

Ability to effectively manage program data in athenaNet, REDCap, and/or ODES system. 

Health and wellness experience preferred and public health background, a plus.

Completion of Medical Community Partnerships 101 and 201 e-Learnings in the Y’s Learning and Career Development Center (LCDC) website, as well as effectively maintaining an active instructor certification in a CDC approved DPP curriculum.
