Overview:

Diabetes Nurse: The RN, Certified Diabetes Educator in conjunction with other team members, is responsible for the education component as outlined by the American Diabetes Association Continued Recognition Program Standards: Provide assessment and education to: a) newly diagnosed, b) established, and, c) hospitalized patients with diabetes mellitus seen at the University of Florida, and d) established patients with diabetes mellitus referred to the UF Pediatric Diabetes Clinic. Create and maintain diabetes education material provided to newly diagnosed and established patients. Assess each patient�s knowledge base, skills, comprehension, compliance, and educational needs in order to determine essential education needs required to manage diabetes. Provide telephone, e-mail, EMR-based follow-up of patient lab values and home blood glucose results and communicate results and potential therapeutic adjustments to physician and patient. Responsible for completing forms required by schools for distribution of insulin. Responsible for obtaining pre-authorizations as needed In addition, Diabetes self-management education (DSME) is provided within the scope of practice for RN including participation in insulin pump and continuous glucose monitoring (CGM) programs, including organizing classes for patients, parents, and other medical care-givers and initiating insulin pump and CGM treatment. Responsible for billing and follow up with pharmaceutical companies for insulin pump training. The Diabetes Educator will also participate in diabetes studies in children by being knowledgeable about the on-going studies at the University of Florida and identifying and recruiting appropriate subjects for these studies. Responsible for completing forms required by schools for distribution of insulin. Job includes 5 days/week (Monday-Friday, 8-5).

Dietician: 

Provides clinical nutrition care to patients, demonstrating knowledge of the principles of growth and development over the life span, assessing data reflective of the patients' status, evaluating nutritional status and developing, implementing, monitoring, and documenting nutrition care plans.  Serves as a resource with development of age-specific education materials and instruction of dietetic students.

Qualifications:

Diabetes Nurse Minimum Education and Experience Requirements: Graduate of an accredited School of Nursing. Current licensure by the State of Florida as a Registered Nurse. Current certification as a Certified Diabetes Educator (CDE) as outlined by the American Diabetes Association Continued Recognition Program Standards or eligible to obtain CDE. Person should display initiative, has the ability to be multitasking and be able to take on responsibilities. Proven ability to problem-solve and be a team member is essential. Motor Vehicle Operator Designation: Employees in this position: none provided Licensure/Certification/Registration: Current licensure by the State Florida as a Registered Nurse. Basic Life support certification required or obtained at first opportunity with employment. Certified Diabetes Educator (CDE) eligible.

Dietician Minimum Education and Experience Requirements: 

Must have passed or be eligible to take the ADA Registration exam.  Must be a state of Florida licensed dietitian/nutritionist or be eligible for same.  Within six months of starting employment, must have passed both the ADA Registration and become licensed as a dietitian/nutritionist in the state of Florida.  Otherwise, failure to do so will result in non-continuation of employment.
