How you make a difference:

The Mental Health Counselor conducts comprehensive assessments of individuals, considering both clinical and psychosocial factors. They deliver group and family therapy interventions, make informed clinical and psychosocial evaluations, and coordinate discharge processes autonomously. Additionally, they stay updated on clinical approaches, facilitating effective communication between the treatment team, families, guardians, and community providers.

Key Responsibilities:

• Provides individualized care and treatment, considering age and developmental needs of each person served.

• Develops biopsychosocial summaries, implements interventions for discharge barriers, and coordinates all aspects of discharge planning and documentation.

• Advocates and negotiates with internal and external systems on behalf of the person served consistent with professional conduct and standards.

• Independently provides all care activities and appropriately seeks direction for unusual situations.

• Independently provides crisis intervention and assists patients in understanding the complexities of mental illness.

• Utilizes all persons served contact as opportunities for empowering therapeutic interventions. Consistently attends and represents clinical needs of persons served during community meetings and treatment team meetings.

• Provides counseling, brief interventions, and facilitates therapeutic groups in accordance with patients treatment plan objectives and needs.

• Updates all programmatic tracking lists, documents all directive services and collateral contacts in EHR in a timely fashion. Develops and updates treatment plans in an accurate and thorough manner.

Qualifications & Requirements:

Education

A Master’s Degree in Mental Health Counseling is required.

Experience

Must have knowledge of the DSM-IV - Diagnostic and Statistical Manual of Mental Disorders and community resources.

Two (2) years of experience with the severe and persistent mentally ill. 

Licenses/Certifications

Licensed Mental Health Counselor is preferred.
