Overview:

The Social Worker is responsible for providing high quality patient care, displaying good skills in assessment, treatment planning, group treatment, individual treatment, family treatment, and discharge planning as well as function as a member of the multidisciplinary team.

Responsibilities:

Knowledge of social work programs & community services, & ability to coordinate linkages with community resources & providers of service to ensure continuity of care at the point of discharge.

Knowledge tn the care and handling of residents of all age groups and has knowledge of the special needs and behavior of each respective age group.

Demonstrates the ability to conduct thorough & comprehensive intake assessments & psychosocial histories, to identify significant problems to be addressed in the treatment plan & discharge plan & document progress in medical records.

Demonstrates the skills necessary to provide emotional support to the patient & family to cope with the stress of psychiatric hospitalization, & separation from family & home.

Documents significant contacts in the progress notes during the treatment consistent with designated timeframes.

Participates in daily review/rounds of all patients with members of team.

Participates in weekly treatment plan meetings and completion of the Comprehensive Treatment Plan and Treatment Plan reviews.

Ensures Continuity of care at the point of discharge, as documented on the Discharge Summary.

Coordinates linkages with community resources and providers of service

Understands the need, and has respect for, the cultural and socio-economic diversity of our patient population.

Qualifications:

Education:

Master's Degree in Social Work form an accredited school, required.

Experience:

Minimum of one (1) year clinical experience in outpatient or inpatient social work.

Knowledge and Skills:

Ability to communicate effectively in English both verbally and in writing with patients and co-workers. Bilingual preferred but not required.

Ability to hear, talk, walk, stand, bend and stretch

Basic computer knowledge.

Ability to read, write and interpret documents.

Must be trained in and knowledgeable of all Kingsbrook/Rutland policies and procedures pertaining to hazardous waste and hazardous materials and maintain qualifications applicable to assigned work.

Licenses, Certifications:

New York State Social Worker License (LMSW/LCSW) required. Acceptable upon hire for one year: valid NYS Limited Permit (LP), or eligible and receives LP within 60 days of hire.

Physical Requirements:

Involves standing for long periods of time at required patient bedside, walking, sitting for long periods of time at a desk, talking, hearing and bending.

May include some repetitive motions.

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position without compromising patient care.

May work in surroundings with degree of dust, noise, etc. without undue discomfort.
