Job Title: Licensed Practical Nurse (LPN) - Developmental Disabilities Residential Care
Location: Multiple residences within the Philadelphia area
Shift: 3 PM - 11 PM
Salary: $35 per hour
Type: Temp to Perm, W2 position with full benefits

Summary:
The ideal LPN candidate will oversee, coordinate, and directly address health-related concerns of program participants. This role requires a strong sense of responsibility, dedication to patient care, and the ability to travel to different locations throughout their shift.
Essential Duties and Functions:
Maintain accurate and current nursing documentation and medication administration records.
Educate and assist residents with medication management and adherence to physician orders.
Provide various nursing care services, including but not limited to wound care, trach care, and diabetic counseling.
Assess the health and emotional needs of residents and adjust care plans accordingly.
Communicate efficiently with the nursing team, program staff, and management regarding resident concerns.
Uphold the highest level of confidentiality in all aspects of the job.
Credentials Required:
Valid Licensed Practical Nurse (LPN) license.
Pennsylvania Criminal Clearance (obtained within the past year).
Current CPR certification.
Proof of Covid-19 Vaccination.
Valid driver's license.
Recent Physical and TB or Quantiferon test (within the last 6 months).
