Contract/Project
To support the basic needs of people with profound and multiple learning disabilities in line with their support plans and guidelines.

To support people with profound and multiple learning disabilities in sessions to achieve the outcomes they want as specified.

1. To ensure high levels of dignity are maintained whilst supporting people to eat, drink, have personal care and any other support needs met in line with the guidelines provided.

2. To follow the eating and drinking guidelines for each individual carefully and report any concerns immediately to the therapy coordinator.

3. To Support and Maintain people’s physical wellbeing through good manual handling practices,

4. To Maintain peoples physical welfare by following all OT and Physiotherapy guidelines and supporting the OT and Physio Coordinators to deliver therapeutic movement sessions.

5. Work alongside the Therapy Coordinator to continue to develop your own creative communication skills with people who have profound and multiple learning disabilities

6. Be able to adapt your communication style and approach with each adult depending on their personalised communication guidelines.

7. To contribute towards the personalised support plan by advocating on someone’s behalf and utilising creative communication where appropriate.

8. To provide written updates such as; updating families and carers on daily activities and events, daily plans and risk assessments.

9. To understand and to implement the Councils Safeguarding of Vulnerable Adults policy.

1. To support people to access the service safely via any means of Transport identified in the person’s support plan; escorting, support on and off the vehicles, ensuring wheelchairs and mobility aids are secure, door to door service with handover to an appropriate care, utilising local transport, walking with wheelchairs to and from activities and home

2. To ensure all financial protocols are followed when handling petty cash.

3. Maintain general indoor housekeeping of the Community Hub
