The Community Health Worker provides social support, navigation and care coordination to patients in pediatric primary care practices of HMH. CHWs help patients address the social, medical, and community problems that lead to poor health. The goal is to help patients and their families deal with the real-life issues that keep them from staying healthy and to help the families by assisting with their care management. CHWs are in regular communication with the pediatric primary care clinicians and staff.

This is a part-time, 2 year grant-funded position with the goal of long-term sustainability. It has a hybrid work schedule where you will be working onsite in the Rockleigh, NJ office  one day a week and the other days, you will be working at home or in the community.  Reach out to pediatric patients and their families to address any barriers to their health. Act as a liaison between the patient and the office and connect them with suitable resources.  

Responsibilities:

A day in the life of a Community Health Workers (CHWs) at Hackensack Meridian Health includes:

When possible, meet patients in the primary care office to introduce themselves to the families they will work with. This includes patients with chronic conditions, behavioral and/or social problems.

Provide patients with emotional, informational, and instructional support. 

Motivate patients to meet their health goals.

Make weekly follow-up calls to patients.

Work with the clinicians and the family to develop concrete written plans and review them with the families.

Provide resources to help families with social issues like home and food inequity.

Develop resource guides of community agencies to include daycare for use of the office staff.

Develop working relationships with core community agencies. 

Help families connect with community resources; help families complete applications as necessary.

Help families with transportation needs and scheduling appointments. 

Assist caregiver and families with navigating health care and social service systems

Finding patients lost to care.

Work with other CHW's in pediatric practices.

Attend community events to promote child health.

Attend regular practice team meetings.

Other duties and/or projects as assigned.

Adheres to HMH Organizational competencies and standards of behavior

Qualifications:

Education, Knowledge, Skills and Abilities Required:

Bachelor's degree in health-related field or 3+ years experience in the health or hospitality field with a High School. diploma, general equivalency diploma (GED), and/or GED equivalent programs.

Minimum of two years of experience in health or hospitality.

Ability to act professionally when working with families, staff, and community agencies.

Excellent written and verbal communication skills.

Ability to carry out written communication skills.

Ability to exercise judgment in the application of professional services. 

Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.

Knowledge of social services resources available to the community.

Education, Knowledge, Skills and Abilities Preferred:

Prior experience as a community worker a plus.

Prior experience working with pediatric-aged patients a plus.

Licenses and Certifications Required:

Valid Driver's License from a USA state.
