REQUIREMENTS:
Required: Doctor of Pharmacy required from an accredited institution.
Required: Completion of PGY-1 residency in an acute care setting with additional 2 (two) years of clinical experience or completion of PGY-2 residency.
Preferred: 2 years Residency training.
Required: Licensed to practice in the State of Texas.
Required: Completion of an accredited Residency in Hospital Pharmacy, specialty training in Disease State Management.
Required: ACPE IV certification.
Preferred: Antimicrobial stewardship certification.
Required: ACLS certification.
Required: AHA BLS.
Encouraged any Board of Pharmacy Specialty certifications.
