Responsibilities:
Responsible for the coordination, continuity and quality of clinical care following a physician approved plan of treatment.
Facilitates the completion of documentation to comply with regulatory, operational, and financial requirements.
Acts as a clinical resource to internal and external customers.
Has responsibility for managing associate performance, development and coaching of assigned clinical associates.
Qualifications:
Graduate from an accredited school of nursing
Licensed RN in Minnesota
Two years RN experience with one year in Home Care
Two years management or supervisory experience in a healthcare setting, preferably Home Care
Preferred
BSN degree
Hospice experience
