POSITION SUMMARY:

The Physical Therapist Assistant is trained and licensed, meeting the qualifications set forth by the NYS Board of Education and the NYS Rules and Regulations requirements. The Physical Therapist Assistant assists in providing therapy services to patients including treatment implementation, related documentation, and communication as directed by the Physical Therapist.

CORE QUALIFICATIONS:

1. Education:

a. Graduate of an accredited school of Physical Therapy Assistant.

b. Licensed in NY State as a Physical Therapist Assistant.

2. Work Experience:

a. Minimum of one-year experience as a PTA in a skilled nursing facility setting

3. Job Knowledge/Requirements:

a. Able to read and interpret documents such as safety rules, operating and maintenance instruction, and procedural manual.

b. Aptitude with digital documentation software, email, digital billing, and utilizing hand held devices, lap tops, PC's and IPADS.

c. Competency with basic computer software Word, Excel, Outlook.

d. Manage stressful situations, deadlines and challenging patient caseloads.

e. Ability to deal with resident and family concerns

Requirements:

ESSENTIAL JOB RESPONSIBILITIES: During the course of your shift you may be required to perform some or all of the following:

1. Performs treatment programs for assigned residents.

2. Enter billing information in an accurate and timely manner.

3. Report resident progress in meetings, i.e. UR Meeting, Patient/Family Conference as deemed required or appropriate.

4. Consult with other members of the rehabilitation team to coordinate physical therapy with other therapeutic activities.

5. Provide residents with equipment as appropriate.

6. Inform Social Services of equipment needs at discharge.

7. Design and construct special equipment for individual and suggest adaptation of individual's living environment.

8. Work with the Director of Rehab to request supplies and equipment.

9. Lay out materials for individual's use and returns equipment to appropriate place.

10. Conduct training programs for other workers/families in physical therapy techniques and objectives for resident care.

11. Chart notes in an informative and descriptive manner that reflects the therapy provided to the residents, as well as the resident's response to the therapy.

12. Perform routine documentation as required and in accordance with our established charting and documentation policies and procedures. Sign and date all entries made in residents' medical record.

13. Cooperate with all departments and staff facilitating harmonious working conditions to promote optimal resident care.

14. Follow established safety procedures and precautions; observe and report hazardous conditions for correction; report fall accidents/incidents promptly.

15. Observe resident's rights as defined in the facility.

16. Ensure that all nursing care is provided in privacy.

17. Report all grievances and complaints made by the resident.

18. Maintain a written record of the resident's complaints and/or grievances that indicates the action brought to the appropriate person.

19. Adhere to all facility and department policies and procedures.

20. Ensure all HIPAA and Corporate Compliance requirements are adhered to.

21. Perform other related duties as required and assigned.

ADA Requirements:

Candidate will have the physical ability to perform job related duties, which may require lifting, bending, stooping, stretching, pulling and pushing up to 50 lbs or more as needed.

Pay - 33.53/ hr
