Overview:

The Patient Services Navigator is responsible for the assessment, evaluation and management of patient problems, such as personal or financial issues. This role develops a treatment plan that meets the assessed needs of the patient, collaborating with internal and external resources. The schedule for this position is Monday - Friday, 8:00am - 4:30pm.

Talent Pool - Administrative & Clerical

Responsibilities:

Collaborates with physicians, nurses and other hospital departments to help address patient/family needs, creating a treatment plan for the patient.

Evaluates the personal, social, emotional and financial needs of patients and families to help limit the impediment of these needs to the effective treatment of the patient, increasing their rate of recovery and chance of successful treatment.

Serves as a liaison for families and patients with hospitals, community and third party resources to assist the patient in navigating what can sometimes be a complex treatment plan.

Maintains knowledge of and contacts external agencies such as care facilities and community resources to ease the patients treatment post-discharge, setting them up on a path of future success.

Engages with the clinical care team to ensure that documentation and communication address the limitations of the patient and family, communicating clearly and effectively with clinical staff.

Performs other duties as assigned.

Qualifications:

EDUCATION:

Required: Bachelor's Degree of Social Work 

EXPERIENCE:

Required: Three to five years' experience in patient navigation or case management.
