Connections at Home via Willow Valley - Supported by Willow Valley - is the regions' premiere Lifecare Community for more than 30 years. We're seeking a Clinical Services Supervisor to join our extraordinary Connections at Home team! Our Connections at Home Clinical Services Supervisor will assist in implementing policies and procedures and assist in the managing of all clinical services and operations and regulatory compliance.

Responsibilities:

Assists in the managing of Connections at Home’s clinical operations to meet budgeted revenue and expense goals.

Ensures that Connections at Home Team Members and representatives abide by accepted standards of practice as set forth by all related regulatory entities.

Assists in the supervising of the work force to ensure that high quality care is provided to clients and that Connections at Home is successful. Ensures that Connections at Home and Willow Valley Communities’ policies and procedures are followed and enforced.

Assists in maintaining a high level of competence within the team by accessing and implementing training and educational programs inside and outside of Willow Valley Communities.

Assists in developing positive relationships with referral sources, i.e. physicians, hospital discharge planners, clergy, care managers, other home care organizations, and social workers to facilitate a steady stream of referrals of new business to the organization. Works with the appropriate Willow Valley Communities Team Members to ensure internal referrals.

Assists with the implementation of the marketing and advertising plan which may include newspaper advertising, phone book, fairs, distribution of printed materials, managing inquiries, etc.

Oversight of and assistance with the scheduling of clients to ensure that all clients requesting services receive what they need in a timely and effective manner.

Assists with the management of medication dispensers and/or other equipment as deemed necessary.

Recommends new services and/or lines of business to develop and maintain a healthy organization as deemed necessary.

Delegates authority to others for continued management of services and scheduling during absence from work.

Conducts initial assessments and creates the plan of care for new clients and/or new services.

Directs the intake of new clients to ensure that all requests for services are handled as promptly as possible

Assists in maintaining Connections at Home in good standing with all authorities having jurisdiction over the organization and assists with processing all applications for licenses and permits.

Ensures that documentation of services is accurate and timely.

Assists in implementing and supervising the administrative policies of the clinical department and administratively supervising the provision of all services.

Assists in managing a quality improvement program. Supervises and evaluates client satisfaction survey reports on clients served as deemed necessary.

Is available via telephone during the agency’s operating hours and assumes on-call responsibilities. When on-call, carries a cell phone at all times and manages all phone calls into the office. This may include intake and managing call-outs and/or no-call/no-shows

Fills in as a caregiver whenever necessary.

Supervises the Office Supervisor and has direct supervision of all Team Members including caregivers and companions.

Works closely with various department Directors of Willow Valley Communities who provide services to Connections at Home.

Qualifications:

A LPN or RN with at least five years of clinical experience with at least 2 years with the geriatric population.

Minimum of 5 years of clinical experience preferred.

Valid Driver’s License

We serve clients located in their private homes through-out Lancaster County and Willow Valley Communities.
