RN- Palliative Care (At Home)
NOT YOUR TYPICAL HOME HEALTH POSITION
NO ROUTINE SKILLED NURSING TASKS REQUIRED
Typical Visit: Often shared with our Palliative Care Nurse Practitioner, routine nursing assessment (vital signs, focused palliative review of systems, physical exam, goals of care conversations)
Days 8a-5p Mon-Fri (No weekend, vacation or call)
Multiple locations available: West Nashville, Clarksville, Shelbyville and Dickson

$38-$43/hour (Guaranteed 40/hrs weekly)

Responsibilities:
Performs visits for an assigned caseload of patients and families in their homes to complete guided assessments, support medication management, address complex symptoms and coordinate care.
Engages patients and caregivers in discussions about their goals of care, reviews advance directives and collaboratively develops treatment plans based on goals.
Monitors care plans in conjunction with the rest of the clinical care team.
Evaluates medical needs for an assigned caseload of patients and provides them complex skilled care.
Communicates plan of care to primary physicians and other consulting clinicians.
Educates patient/family on trajectory of illness, use of medications and assesses disease understanding.
Performs all other duties as assigned.
Qualifications:
Associates degree in nursing.
Licensed as an RN, specific to the state(s) you are assigned to work.
Two years of experience managing patients with medically complex and chronic illness.
Preferred
Bachelor's degree in nursing.
Certified Hospice and Palliative Nurse (CHPN), BCLS, and/or ACLS certification.
Homecare, hospice or palliative experience.
Chronic illness management experience and/or advanced care planning skills.
