The Care Manager is responsible for coordinating delivery of services to guests in collaboration with the facility’s team members.

Responsibilities:

• The Care Manager monitors and documents the effectiveness of treatment provided, coordinates the admission process and determines with the interdisciplinary team (IDT) appropriate post discharge services.• Coordinates and updates Care Plan. Attends meeting if requested by guest or deemed appropriate by IDT.• Participate in daily IDT meeting, provide clinical review as necessary.• Complete assigned sections of Discharge Plan Instructions. Review clinical post discharge instructions with guest upon discharge.

Qualifications:

• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be agraduate of an approved LPN/LVN program.• Must possess a current, unencumbered, active license to practice as an RN or LPN/LVN throughthe State Board of Nursing.• Must have a minimum of two (2) years’ experience in a hospital, nursing care facility, or otherrelated health care facility.
