We are currently looking for a PRN Licensed Practical Nurse (LPN) Intake Coordinator to join our Intake team.

Responsibilities:

Illustrative duties may include, but are not restricted to, one or more of the following:

Provides education and information regarding VNA programs, policies and services to the local community, healthcare facilities, physicians, patients and their families.

Works closely with skilled nursing facilities, physician’s offices, the community and VNA staff to facilitate the patient transition to home care.

Maintains and develops contacts and knowledge of the resources that are available within the local community.

Ability to use knowledge and skills to convert informational calls into home care referrals.

Ability and commitment to meet monthly budget numbers.

Responsible for the entry of patient information accurately and timely as part of the electronic medical records system.

Answers all incoming calls in a timely manner.  Effectively manages calls from external referral sources.  Maintains an accurate record of the origin and the response to all incoming calls.

Identifies unmet learning needs and seeks assistance from others in meeting those needs.

Complies with company policies and procedures.

Completes all required forms, reports, time cards, itineraries, etc, accurately and within designated timeframe.

Recognizes and follows established lines of communication.

Performs other duties as assigned.

Qualifications:

High School Diploma or GED.  Graduate of an accredited LPN program.  Associate's degree in Nursing preferred.

1 year hospital experience, preferred

Home care and/or Community Health experience is preferred.

Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.

The ability to work in a complex institutional structure requiring significant collaboration. 

Ability to work independently with minimal supervision.

Demonstrated ability to maintain strict confidentiality. 

 Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact
