Chaplain Bereavement Coordinator

Home Health Hospice

Fort Dodge, IA

3 days/week - Monday-Friday 8AM-4:30PM 

24 hours/week

Part-time + benefits

Responsibilities:

The Bereavement Counselor cooperates with the bereavement team and other hospice staff and volunteers in developing a comprehensive. range of services to: inform, support, and guide patients and families through the process of bereavement before, at the time of, and after the death of a loved one; educate, train, and support individuals in their professional or volunteer roles as bereavement care providers; and anticipate and respond to community education and supportive needs related to bereavement.

Plan, implement and maintain and present bereavement related programming to meet the needs of families/caregivers, as well as regulatory requirements for the Medicare Conditions of Participation.

Develop educational programs and materials for patients, families, staff, and the community on loss, grief, and coping with bereavement.

Plan, implement and facilitate bereavement group events.

Prepare service reports as requested by clinical manager.

Evaluate bereavement program services annually and provide recommendations for development, improvement, or expansion.

Facilitates the development of the interdisciplinary team plan of care to meet individual spiritual needs.

Maintain record of spiritual care services and related activities for quality assurance, program development, and policies and procedures.

Document direct services and ongoing communication with community chaplains, spiritual counselors and hospice interdisciplinary team.

Qualifications:

Graduate of accredited school of theology, or related Bachelor’s degree. 

2 units of CPE

Ordination in a particular religious group and/or endorsed and in good standing from an ecclesiastical governing body

Valid driver’s license when driving any vehicle for work-related reasons.

CPR Certification.
