Responsibilities:

The Community Health Worker will work primarily with specific target populations including but not limited to non-English speaking populations of geographic target areas. The Community Health Worker will work closely with internal and external stakeholders or partners including but not limited to clinical/medical/dental providers, primary care teams, and other agencies to improve patient care and outcomes. Patient outreach interactions may be telephonic, video/digital, or in-person generally at an office site. Depending on department Community Health Workers may need to be bilingual or be knowledgeable of specific department terminology such as medical or dental.
