Overview:

Fairview Health Services is seeking a physician to work in partnership with operation leaders to provide overall leadership for Care Map program services across the system. The Program Medical Director is 0.2 FTE and up to 0.8 FTE in a clinical practice.

Job Expectations:

Works with the Care Map Program Manager, CVR System Director, Care Map Medical Directors and operational leaders to set the strategic direction and goals of the Care Map Program

Develops and implements clinical protocols/tools consistent with best-known practices with CQVC team

Communicates, educates and assures compliance with the use of these protocols/tools

Partners with other Medical Directors and broader leadership team to support the strategic priorities and change initiatives of both the division and the organization

Consistently seeks out opportunities to expand knowledge of best practices and new developments within related areas and incorporates this knowledge into the operation of the program

Understand evidence and build evidence-based protocols with teams

Utilize data to make decisions and help prioritize care map program priorities

Understand and collaborate with clinical operations on clinical workflows to support care maps

Drive care map engagement with clinical teams, as well as medical, clinical, and operational leaders

Qualifications:

Required:

Graduate of an accredited ACGME/AOA residency training program.

Board Certified with the ABMS or AOA

Licensed or ability to obtain Minnesota Board of Medical Practice Licensure.

Ability to obtain and /or maintain DEA certification for the State of Minnesota.

Meet threshold criteria/qualifications for Credentialing and Privileges.

Three years of clinical experience

Preferred Qualifications:

3 years of experience in a leadership role or with experience partnering in leading and implementing systemwide projects

Experience in quality improvement

Experience with evidence standards and/or protocols

Experience with data-driven decision making
