Responsibilities:

The Licensed Practical Nurse provides intermittent skilled nursing services to patients in the home setting according to the patient’s plan of care. As a Licensed Practical Nurse you will:

Perform agreed-upon tasks under the patient’s plan of care

Report significant findings to patient's physician and RN as they occur

Schedule a Case Conference/IDG with assigned RN to review patient's needs, problems, level of care and any changes in plan of care, including hospice admission with patient consent

Participate in Quality Assessment and Performance Improvement (QAPI) program

Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

Qualifications:

Qualifications

Currently licensed as a licensed practical nurse by the State Board of Nursing

Must maintain a valid driver’s license, reliable transportation, and good driving record

Ability to work in a field setting and exhibited ability to make sound nursing judgment

Proficiency in Microsoft Office
