Hatched Recruitment is working closely with the local authorities and we are looking for a Community Care Worker in the Hillingdon area.

The post holder will assist in conducting statutory Care Act Assessments for Older People and clients with Physical Disabilities, aiming to meet the 28-day performance targets and reduce waiting times for new referrals.

They will manage long-term, complex cases, which may involve hoarding, non-engagers, DoLS challenges, and court of protection work (e.g., welfare disputes, court applications). Responsibilities include: Completing six-week reviews Conducting Mental Capacity Assessments Performing Continuing Healthcare assessments

You will be an experienced Community Care Worker with knowledge of working with Older Adults and/or People with Disabilities. You will have enhanced your skills and knowledge through further training and possess specific experience with the relevant client group.

You will be innovative and creative, willing to explore new working models to improve outcomes for service users. Additionally, you will be a highly motivated individual capable of working as part of a team, building strong relationships to deliver the best possible outcomes for vulnerable people
