Locum Community Nurses – Bradford - £28+ p/h

* £28+ per hour

* Full-time hours

* Initial 3-month contracts

We are seeking dedicated and compassionate Locum Community Nurses to work with the community healthcare teams in the Bradford area. The successful candidates will provide high-quality nursing care to patients in their own homes or within community settings, focusing on promoting health, preventing illness, and managing long-term conditions. This role requires strong clinical skills, excellent communication, and the ability to work independently while being part of a multidisciplinary team.

Key Responsibilities

Patient Care

* Provide holistic nursing care to patients in the community, including those with chronic illnesses, post-operative needs, and palliative care requirements.

* Perform clinical assessments, wound care, medication administration, catheter care, and other nursing procedures.

* Monitor and manage patients with complex health conditions, ensuring their care plans are followed and updated as necessary.

* Educate patients and their families on disease management, medication adherence, and healthy lifestyle choices.

Care Coordination

* Work closely with GPs, social workers, physiotherapists, occupational therapists, and other healthcare professionals to coordinate patient care.

* Participate in multidisciplinary team meetings to discuss and plan patient care.

* Facilitate referrals to other healthcare services as needed.

Patient Education and Support

* Provide emotional and psychological support to patients and their families, addressing concerns and ensuring they feel supported in managing their health.

* Offer advice and education on managing long-term conditions, wound care, and prevention of complications.

* Empower patients to take an active role in their own care through education and self-management strategies.

* Documentation and Reporting

* Maintain accurate and up-to-date patient records, including care plans, progress notes, and medication records.

* Ensure all documentation meets legal, ethical, and organizational standards.

* Report any changes in patient condition or concerns to the appropriate healthcare professionals promptly.

Health Promotion and Prevention

* Participate in health promotion activities within the community, such as vaccination programs, health screenings, and education sessions.

* Identify and address potential health risks in the community, promoting preventive measures.

Qualifications and Experience

Essential

* Registered Nurse qualification with current registration with the Nursing and Midwifery Council (NMC).

* A minimum of 2 years of post-registration experience, preferably with experience in community nursing or a similar setting.

* Strong clinical skills, including wound care, medication administration, and the ability to manage complex care needs.

* Excellent communication and interpersonal skills, with the ability to build rapport with patients and families.

* Ability to work independently and manage a caseload effectively.

* A valid driver’s license and access to a vehicle for home visits (if applicable).

Desirable

* Experience in palliative care, chronic disease management, or post-operative care.

* Additional qualifications or certifications in community health, chronic disease management, or related areas.

* Previous experience in a locum or temporary nursing role
