Qualified Social Worker - Children with Disability Team (East)

Location: Westport House, Wareham

Full-Time (37 hours per week, normal office hours)

Children with Disability Team East Job Summary:

We are seeking a Level 3 Qualified Social Worker to join our Children with Disability Team at Westport

House, Wareham. The role involves supporting children with disabilities and their families across East Dorset and Purbeck. You will assess needs, develop care plans, coordinate services, and work with multi-disciplinary teams.

Key Responsibilities:

Assessment and Planning:
Conduct comprehensive assessments and develop care plans.
Manage a caseload of up to 18 children (CP, CIN, CiC cases).
Support and Advocacy:
Provide direct support and advocate for children and families.
Coordinate short breaks for parents and carers.
Service Coordination:
Collaborate with health, education, and social care professionals.
Engage in multi-agency working with Health, SEN, ID CAMHS, CAMHS, and MH settings.
Safeguarding:
Handle safeguarding concerns and conduct Section 47 investigations.
Apply the Mental Capacity Act for children aged 16+.
Documentation and Reporting:
Maintain accurate case records and reports.
Present cases to funding panels.Qualifications and Experience:

BA(Hons) in Social Work.
Registered with Social Work England (SWE).
Minimum three years post-qualifying experience in a Local Authority.
At least one year working with disabled children or young adults.
Knowledge of the Mental Capacity Act and short break procedures.
Experience with complex cases involving mental health, learning, and physical disabilities, including autism.
Valid driving license with business use insurance.Additional Requirements:

Ability to work both in the office and remotely.
Enhanced DBS check.
Current SWE registration number.
Availability ASAP.Team Structure:

Team includes a Team Manager, five social workers, one student social worker, and one family worker.Opportunities:
