Job Summary:To further the Agency's mission by providing nursing services under the direction of the RN case manager and according to the Plan of Care as written. To evaluate and report to the RN Case Manager the effectiveness, progress, or any unfavorable changes in patient condition.
Responsible for the supervision of Home Health Aides to ensure that the Home Health Aide Plan of Care is being followed. Report to the Manager of Private Duty.
Duties and Responsibilities:
Under the direction of the RN Case Manager, completes nursing visits and provides clinical services as determined and authorized by the client plan of care.
Supervises all patients care according to physician orders and within required time frames
Responsible for timely completion of clinical documentation, communicating status changes and care plan progress to all corresponding disciplines.
Responsible for completing physical assessments within the time frames dictated by the regulating bodies
Provides clinical education and resources to clients and families within the community
Travels throughout the geographic market to provide nursing services and to supervise and evaluate homemaker/PCA staff
Participates in case conferences with the program director, as needed
Licensed Practical Nurse
2 years prior experience in homecare with Medicare homecare experience a plus
Valid LPN license in Ohio
Current Ohio Driver's license
Current TB test or chest x-ray
Current CPR certification
Current car insurance
Knowledge, Skills and Abilities:
Ability to organize a large quantity of information
Knowledge of Medicare and Medicaid regulations
Proficient in Microsoft Word, Excel, E-mail and ability to learn other software programs associated with position
Ability to drive and have reliable automobile
Excellent customer service skills
Knowledge and adherence to HIPAA regulations
Ability to communicate the mission of JFSA
Excellent communication skills
Ability to work as a team member
Excellent organizational and time management skills
Ability to be flexible in schedule and to work in a stressful environment
The physical demands may include instruction and assistance to Home Health Aides in transfers and lifting
