I am working with an ICB in South East England who are looking for a qualified Learning Disability CHC Practitioner. Must have Continuing Health-care and ICB experience and be registered on NMC.

Successful interim must be immediately available & able to do a mix of virtual and face 2 face meetings.

They require an agency person to help carry out & support to clear reviews. This post will largely be for the CHC reviews and immediate case management/actions resulting from reviews.

• Day to day expectations: Review allocated clients (annual CHC/S117). Carry out actions resulting from review & right size package of care.
• Start date: ASAP.
