Primary Location: IA > DES MOINES > MERCY MEDICAL CENTER
Coordinates and provides care forgroups of high-risk individuals and/or populations in the inpatient and/orcommunity setting. Assists other healthcare personnel in identifying health care needs. Interacts with patients and families and alllevels of personnel, medical staff, community services, and insuranceproviders. Identifies and coordinateseducational programs for patients and health care professionals, as well as thecommunity. Interprets standards for thecare of a specific patient population and provides clinical, consultative,educational and research support to ensure that quality cost-effective care isconsistently provided. Communicatesacross health care areas to ensure that systems integrate efficiently in orderto best serve patients, staff, physicians and community.

In collaboration with multidisciplinary team,evaluates patient referrals to develop individualized plan of care across thecontinuum of care
Responsible for personalized education plan forpatient and family
Follows through with test results and plan ofcare with patient/family and other health care providers/services as directedby provider or established guidelines
Acts as a liaison among the individual/family,community, provider and physician as well as other health care team members.
Collaborates in developing, implementing andevaluating teaching/learning strategies for client/family in the hospitalcommunity.
Provides clinical leadership by being a preceptorfor visiting professionals, students and new graduates.
Coordinates appropriate educationalmaterial/instructions for patients, families, medical staff andmultidisciplinary personnel to accomplish patient care outcomes.
Participates in planning and presentation ofcontinuing education programs, seminars and meetings.
Coordinates with the health care team to developa mechanism to monitor patient outcomes and follows up on variances.
Ensures collection and input of riskstratification data. Reports results tothe health care team and participates in interpretation of data and formulationof action plan.
Collaborates with patients/families/populationsto coordinate community services and appropriate resources which meet theindividual needs and goals.
Serves on selected hospital, group practice andcommunity-based committees..
Identifies need for and participates in researchprojects relevant to specific patient population.
Collaborates with Quality Management,Utilization Review, Information Technology Services, and other departmentsregarding data availability and reports.
Is accountable for the implementation andcommunication of standards for patient care delivery appropriate to thespecific patient population that meets professional and regulatory standards.
Coordinates the health care team in developmentof clinical tools for optimal outcomes and reports findings.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Current Iowa registered nurse (RN) license.
Bachelors degree in health-related fieldrequired or actively pursuing (BSN preferred).
Demonstrates competency within a specializedarea of clinical practice with 4 years experience in that specialty.
Demonstrates success in positions of leadership.
