NURSING PROCESS AND CLINICAL PRACTICE SKILLS
Fully competent in primary nurse role at SMC; performs patient and family assessment & problem identification, plans care accordingly, implements and evaluates plan of care so that problems are resolved.
From problem based assessment, collaborates with MD on plan of care and implements plan of care.
Records pertinent clinical documentation in accordance with regulatory requirements, professional standards and hospital policy and scans documents appropriately. At SMC, documentation includes nursing assessment form and patient education.
Prioritizes patient situations and takes appropriate action in an efficient, effective manner
Responds to clinical needs of patients in accord with experience, nursing standards, and hospital/nursing policies and procedures.
Makes value-added resource decisions for patient care with respect to supplies, equipment, and acuity of care
Provides individualized patient and family education consistent with patient's learning style, readiness to learn and self care needs
Evaluates the effectiveness of patient learning
Coordinates with all members of the clinic team in order to address patient care, resolve problems, and improve the delivery of multidisciplinary care on a daily basis and facilitates work flow in the clinic.
Applies individual and group problem solving; shares knowledge with others freely, consults openly, and assists others to interpret data
Uses verbal and written communication methods effectively
PATIENT SAFETY & CONFIDENTIALITY
Adheres reliably to all accepted patient safety standards and assists in maintaining a clean, safe environment of care.
Identifies and promotes new learning around changing patient safety standards of practice

PROFESSIONAL DEVELOPMENT & INTEGRATION

Demonstrates role awareness and seeks individualized/department specific continuing education that improves patient care or professional practice. Department specific areas of learning focus may include nursing/medical care for non emergent illnesses and injuries; splinting, bracing, and casting; EMTALA; and communicable diseases.

Acts as a relief charge nurse when assigned
VII. UNIT SPECIFIC DUTIES & RESPONSIBILITIES
Demonstrates competency in all aspects of day to day operations of the clinic including patient care, triaging phone calls, obtaining POC labs, obtaining interpreter as needed, patient education, assisting MD and anticipating patient care needs, preparation and processing of medical records, and facilitating follow up care. Assists with registration as needed and ensures appropriate charges are entered for patient care. In addition, assists in maintaining work flow and efficiency in the clinic by communicating with the charge nurse or director issues/concerns/supply needs and problem solving operational needs and equipment issues.
Medication Management: maintains proper documentation of in clinic and discharge meds including proper pyxis use and reconciliation of count to record keeping. Notifies charge nurse or clinical director of OP clinics when medications are low and need to be reordered. Labels all discharge meds appropriately.

Ensures developmentally appropriate care for all patient populations presenting to SMC for care.
