Full time Clinical Social Worker (CSW) needed for a Re-Entry Intensive Case Management Program. CSW will provide supervision and programmatic oversight. Must have at least six (6) months experience working with Justice-Involved populations, or six (6) months of experience providing direct health, mental health or case management services to complex populations, including clients with chronic medical conditions, mental illness, SUD or experiencing homelessness. Provide clinical intervention as needed for complex cases requiring mental health, crisis intervention, or complex care coordination services. Serve as liaison to partner programs, departments and community agencies. Experience and/or training in the following areas: Re-entry populations; chronic homelessness, outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder services, crisis intervention, family reunification/mediation startegies targeted to formerly incarcerated populations, suicide assessment and prevention, affordable housing and public benefits applications, housing navigation, etc.

Job Specific Requirements Position Requirements

1, MSW (degree in social work) preferred. Master's Degree in psychology may be considered.

2. Possess a Current CPR Certification.

3. Fluency in English and Spanish preferred

4. Experience working with diverse populations in a social service setting highly desirable.

5. Experience in working with Electronic Medical Records preferred.

6. Must be registered with the BBS
