Position Information

The Behavioral Health Counselor is responsible for delivering psychotherapeutic counseling services at our Physician Practices Primary Care offices. The Counselor will have the training and licensure needed to address patient?s behavioral health needs and help coordinate referrals to specialty behavioral health services when required outside of the clinic. The Counselor will use evidence based medicine (EBM) strategies to promote behavior change across a diverse range of patients with behavioral and physical health conditions to include depression, anxiety, addiction, obesity among others. The Counselor will help coordinate care for patients requiring behavioral health services in a collaborative care model with direct communication to the primary care provider led team. The Counselor will use measurement guided patient-centered outcomes that incorporates EBM to improve outcomes for patients leading to cost savings, increased patient satisfaction and improved access to care. The Counselor will also provide expertise in mental health/substance abuse issues to the interdisciplinary team including in-service education and support.

Experience Requirements
:

3 Years Clinical - Mental Health Or 3 Years Clinical - Substance Abuse/Addiction Required

Education Required
:

Bachelor's Degree Nursing Or Master's Degree Social Work Or Counseling Required - And Master's Degree Nursing Or Master's Degree Psychology Preferred

Licensure/Certification Requirements
:

Registered Nurse And Certified Mental Health Nurse Required - Or Licensed Clinical Social Worker Or Licensed Professional Counselor Or Licensed Clinical Addictions Specialist Required - And Certified Notary Preferred
