Senior Social Prescriber - Community Wellbeing Service

The role of a Community Wellbeing Senior Social Prescriber is to support the provision, co-ordination and development of the Community Wellbeing Service within Cheltenham and Tewkesbury, in collaboration with primary and community care practices, Integrated Community Care Teams (ICTs), Community Hospitals, Health Visitors, Voluntary, Community and Social Enterprise (VCSE) Sector and Local Authorities.
The Community Wellbeing Service aims to increase the wellbeing and resilience of the local community and reduce reliance on statutory services through:
- Supporting and enabling people to increase their knowledge, skills and confidence required to take control of their wellbeing, live independently and improve their health outcomes.
- Drawing on and increasing the strengths and capacity of local communities.
- Working together with providers in their localities to ensure a coherent service is delivered.
- Providing support at home and in communities to reduce the need for health, social care and community safety interventions.
