Essential Functions:- Provides leadership and direction to contract service providers.- Develops and implements systems and models of care to ensure high quality, cost-effective outcomes for programs within areas of responsibility. Defines and assesses problems; develops and implements solutions for providers, staff, and contract service providers for aggregate populations.- Provides professional consultation for complex situations to internal providers, staff, members and community service agencies.- Acts as primary liaison among contract service providers, members and Kaiser Permanente systems.- Develops and implements programs for changing organization and member needs.
Basic Qualifications:Experience- Two (2) years of experience coordinating/planning or delivering appropriate care in public/home health/long-term care, skilled nursing facility, rehabilitative/acute care, or hospital discharge planning.Education- Bachelor's degree in a health profession, social work or closely related field.License, Certification, Registration- National Provider Identifier (NPI) and Taxonomy code required at time of hire. Additional Requirements:- Working knowledge of hospital discharge planning, community home health resources, skilled nursing, hospice facilities, and Medicare/Medicaid guidelines.- Facilitation, conflict resolution and negotiating skills are required.- Appropriate licensure or certification.- Examples of preferred health profession credentials are RN, LCSW, Physical Therapist, Occupational Therapist, or similar health specialty.
Preferred Qualifications:- Master's degree preferred.
