Housing Assessment Worker (PT)

We are seeking an Assessment Worker for our Newport client
This is a part time role so you will be working 18.5 hours per week over a minimum of three days including a Tuesday
The purpose of this role is to undertake the assessment process for the generic housing-related floating support service in Newport for people aged 55+. Based within the Supporting People Team, Newport, you will be responsible for undertaking assessments of support needs and risk for people aged 55+. You will also be doing home visits to complete assessments with service users. This is a diverse role.
Answering telephone enquiries, and assisting with recording of support applications for housing related support
You will be responsible for signposting service users to alternative appropriate services where required
Undertaking an initial assessment of risk
Undertaking an initial assessment of support need. The assessment will be robust and detailed focusing on a number of issues that are known to impact on a service users ability to manage and maintain their home
Obtaining consent for personal information to be recorded and transferred between providers as part of General Data Protection Regulations will be vital
You will be required to identify those referrals that are appropriate for Lighthouse 55+ and will work with the Supporting People Teams Floating Support Project Officer to ensure a referral is made to an appropriate support service
You will assist the Supporting People Team to manage the waiting list to the service, which you will review and update on a regular basis
You will assist with any routine administration as directed by the Supporting People Team.
