Job Summary Physical Therapist is responsible for planning and administering physical therapy treatment designed to restore function and prevent disability following disease or injury and promote overall fitness and health of patients. Physical Therapists will supervise Assistants and Aides in their provision of patient care and duties. The Physical Therapist will also perform administrative duties as required. Participates in a team-based approach to care in the Patient Centered Medical Home (PCMH) and participates in activities related to Quality Improvement.

Essential Job Duties 1. Provides initial and ongoing physical therapy evaluations and treatment plans in accordance with physician orders and by assessing and interpreting evaluations and tests results of patients. 2. Assures continuation of therapeutic plan following, discharge by designing home exercise programs; instructing patients, recommending and/or providing assistive equipment; recommending follow-up programs. 3. Promptly and thoroughly documents and maintains clinical and administrative records regarding patient limitations/interventions, treatments, responses and progress to administered therapy as well as discharge summaries. 4. Records treatment encounters daily and completes EMR in accurate and timely manner. 5. Supervises and directs Physical Therapy Assistants and Physical Therapy Aides in direct patient care and related activities as requested. 6. Responsible for performing tasks that are within the scope of his/her educational preparation, knowledge, and permitted by the policies and procedures of GLBHC, and other local, state, and federal guidelines.

Marginal Job Duties

1. Must be able to recognize and respond appropriately to urgent/emergent situations per protocols. 2. Maintain best practices for equipment use and preventative maintenance. 3. Ensures work area, exam rooms and supplies are maintained. 4. Complies with GLBHC policies and procedures. 5. Ensures that patient records are maintained in compliance with HIPAA regulations. 4. Establish and maintain effective working relationships with physicians, staff and management. 5. Maintain regular clinic hours as approved by Manager. 6. Attends meetings as requested. 7. Perform other duties as assigned.

Job Specifications

Education: Graduate from an accredited physical therapy program. Licensure: State of Michigan Physical Therapist license and BLS Certification required. Experience: One year as a Physical Therapist or comparable internship.
