Young Adult Case Manager

Under the direction of the Family and Youth Services Program Manager, The Permanent Housing for Young Adults Project is an intensive service model which provides a rental subsidy and case management services for young adults and their families, with the focus on building the skills to maintain housing.
- Provides services for families in accordance with the Evidenced Based Practices, Critical Time Intervention, Conscious Discipline,Transition to Independence Process, and Trauma Informed Care. The families will receive intensive services, several times per week.
- Conducts community and in-home outreach by establishing and maintaining professional and therapeutic relationships with clients. Establish and maintains a working relationship with participants to ensure access to services by providing a safe, non-threatening and supportive environment and utilizing creative and effective engagement techniques. Ensures the engagement of clients by providing services in a trauma sensitive manner.
- Provides an array of community based, mobile services that address the individualized mental health needs of persons who have a severe and persistent mental illness, chemical dependency and who are formerly homeless. Services include, but are not limited to, ongoing assessment of needs, coordination of services, linkage, symptom monitoring, advocacy, outreach, housing placement, and skill development. Assists in achievement of mainstream resources.
- Promotes recovery and increases the client's capacity to positively impact his/her own environment by assisting clients in the development of interpersonal and community coping skills, and self-management techniques. Assists clients in learning daily living skills. Supports the parent in providing adequate supervision and guidance to their children.
- Educate and link program participants to appropriate services identified in the assessment process.
- Initiates appropriate action and crisis intervention with clients or situations through prompt utilization of community resources.
- Coordinates the development of the individualized service plan (ISP) for each assigned client. ISP must collaborative and include client's own statement of need, a specific objective and measurable steps to achieve the objective.
- Ensures compliance with productivity standards and documentation/record keeping for agency and the project, funding and accreditation requirements by maintaining and reporting accurate and timely documentation and statistical data.
- Ensures the linkage to CPST and Psychiatric Care through FLS or other primary agency. Actively participates in team meetings and works collaboratively with project partners. Participates in DCFS scheduled staffing, reviews and meetings if client is involved in DCFS system.
Qualifications: Bachelor's degree in related field, Experience with complex, multi problem families in an urban setting, valid drivers license with less than 6 points.
