PLACE OF PERFORMANCE - Contractor shall furnish services at the (Minneapolis VA Health Care System, one Veterans Drive, Minneapolis MN 55417.

PERIOD OF PERFORMANCE: 5/1/15-4/30/16

SCOPE OF CARE:

DIRECT PATIENT CARE: 95.0% of the time involved in direct patient care. Contractor shall be responsible for direct patient care and consultation. Contract physician(s) shall be responsible for providing Interventional Radiology procedures, including, but not limited to:

Angiography
Cryoblation
Balloon angioplasty
Line insertion
Chemoembolization
IVC filters
Cholecystostomy
Vertebroplasty
Drain insertions
Nephrostomy placement Gastrostomy
Embolization
Thrombolysis
Dialysis access & related interventions
Biopsy
TIPS placement
Radiofrequency ablation
Biliary intervention
Endovenous laser ablation of varicose veins
Paracentseis\Thoracentesis
Chest Tube Placement
Moderate sedation
CT Angiography (CTA)
Joint aspiration
Lumbar puncture
Fluid sampling
Stent placement (including carotid)
1. Clinic: Contractor physician(s) shall be present on time for any scheduled clinics as documented by physical presence in the clinic at the scheduled start time.
2. Consultation Responsibilities: Contractor physician(s) shall provide consultation with and instruction to referring physicians regarding appropriate indications for interventional radiologic procedures so that the most expeditious and clinically appropriate work-up can be done. Contractor physician(s) shall determine the appropriate course of treatment and communicate in person or by phone with the referring clinicians. Contractor shall enter appropriate CPT code for consultation. Contractor physician(s) shall provide post-procedure evaluation and follow-up and clinical consultation regarding complications of interventional radiologic procedures, and enter a note in CPRS for each
3. Medications: Contractor physician(s) shall follow all established medication policies and procedures. No sample medications shall be provided to patients.
4. Discharge education: Provide discharge education and follow up instructions that are coordinated with the next care setting for all emergency department patients.

ADMINISTRATIVE: 5% of time not involved in direct patient care
Quality Improvement Meetings: The contract physician (s) shall participate in continuous quality improvement activities and meetings with committee participation as required by the MVAHCS Chief of Service, Chief of Staff, or designee.
Staff Meetings/Rounds: The contract physician (s) shall attend staff meetings as required by the MVAHCS Chief of Service, Chief of Staff, or designe
