The VA Medical Health Care Systems, One Veterans Drive, Minneapolis, MN has a need for two, general internal medicine physicians for the period of 05/1/2015 through 10/31/2015 with the possibility of extension.

1. Contractor will be provide physicians who are board certified or board eligible in internal medicine or family practice with an active, current, full and unrestricted license or registration in a state which authorizes the licensee to practice without any change being needed in the status of the license. Valid DEA number is required.

2. Providers will need to be approved by the credentialing office of the Minneapolis VA Medical Center.

3. The physicians will provide primary care to general medicine patients. The approximate number of patients that the contractor will be caring for is 1200 patients.

4. All documentation will be done in the VAs Electronic Medical Record, CPRS. At a minimum, a CPRS note will document the service and will meet the requirements of a Level 3 Evaluation and Management code.

5. Give professional direction to the nursing staff.

6. Every effort will be made to see patients within 20 minutes of their scheduled appointment time either by the provider or under the supervision of the attending.

7. The patient encounter form must be completed electronically at the end of each visit or no later than within two business days of the patient encounter.

8. Participate actively and constructively in Quality Assurance activities (such as peer reviews and medical advisory opinions), including programs designed to meet designated VAMHCS Performance Measures.

9. Enter electronic progress notes which detail the physicians assessment, history, and physical findings, the results for diagnostic testing such as lab and radiology, the physicians plan of care and medication reconciliation.

10. Provide appropriate patient education, including but not limited to information about the patients specific disease or condition; anatomy and physiology, expectations of patient compliance, and medication administration actions and reactions.

11. Be available for informal questions and requests for information for other members of the medical staff or house staff regarding individual patients.

12. Be available to consult with other members of the medical staff via telephone regarding emergency treatment of patients.

13. Comply with all aspects of medical staff bylaws with particular emphasis on medical record documentation.

14. Report all patient safety incidents (such as, but limited to, falls, adverse preoperative events and occurrences) to a designated VA employee (e.g., nurse manager; Risk Management Specialist; and/or the COTR) immediately or no later than 24 hours after the event.
