Social Worker (MSW) - Care Coordination

In collaboration with the patient /family, physicians, interdisciplinary team and external agencies, the Social Worker ensures the patient's progress along the continuum in an efficient and cost-effective manner. The Social Worker is responsible for identifying and initiating the most efficient discharge plan to expedite the patient transition to the next appropriate level of care and to reduce length of stay. This position is the organization's expert in assessing and managing the patient's psycho-social needs
ESSENTIAL RESPONSIBILITIES include:
Coordination with Utilization Management nurses and/or Case Managers for post discharge arrangements and advocacy for the patient in obtaining and negotiating resources to meet patient needs, as necessary. Interviewing patients/families to complete a thorough and timely psycho-social assessment and to evaluate current support systems available to the patient and family. Identifying patient goals. Preparing for and participating in multi-disciplinary rounds on assigned nursing unit for the purpose of updating the team on the discharge plan/needs and to identify potential social work referrals. Completing assessments, per department standards, based upon information from multiple sources including the patient, family, nursing staff, physician, as well as, information discussed in multi-disciplinary rounds.Providing patient/family education, adjustment to illness counseling, grief counseling and crisis intervention.
QUALIFICATIONS AND REQUIREMENTS
Education: Master's degree in Social Work required.
Experience: Licensed Certified Social Work (LCSW) preferred. Minimum of two years' experience in acute care, discharge planning experience with knowledge of community resources preferred.
Licenses/Certificates/Registration: Case Management certification (ACM, CCM) preferred.
1. Current LICSW-Washington State, Licensed Advanced SW-Washington State or Current Counselor Agency Affiliated Registrations within 60days of hire-Washington State required, BLS for healthcare if applicable to location. Required for: outpatient environments (Compass Care, Early Intervention, Transitions, Cleft Lip & Palate, North Star Lodge/Oncology Care, Signal Health & Spiritual Care)
2. Current Washington State driver's license and proof of insurance if applicable to location
3. Cell phone for use during working hours, car for travel to and from home visits during shift, if applicable.
