Work at Home!
Primary Responsibilities:
  • Working with the complex care needs
  • Conducting telephonic triage and assessments
  • making referrals to case management
  • Identifying gaps or barriers in treatment plans
  • Providing patient education to assist with self management
  • Work closely with Case Managers
  • Coordinating care for members
  • Making referrals to outside sources
  • Educating members on disease processes
  • Encouraging members to make healthy lifestyle changes
  • Documenting and tracking findings



Required Qualifications:
  • Current, unrestricted RN license in the State of New Jersey
  • Ability to work Monday-Friday 10am-7pm
  • 3+ years experience in a hospital setting, acute care, direct care experience as an RN
  • and / or experience as a telephonic Triage and assessment RN for an insurance company
  • Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
Preferred Qualifications:
  • A background in managed care
  • Case Management experience
  • Bilingual (fluent in English and Spanish)
  • Ability to work on an auto-dialer call system
  • Experience working with chronic illness and complex care needs population
  • Experience in utilization review, concurrent review and / or risk management
  • Medicaid Experience
