Outpatient Physical Therapist.

Our busy rural Outpatient Physical Therapy clinic is seeking a motivated Physical therapist to join a well established and successful practice located in Lamont, CA. The clinical setting provides a challenging environment to a PT looking to gain or improve their assessment and treatment skills, while developing a sound inventory of autonomous clinical decision making. After nearly 20 years of providing excellent personalized care to a variety of patient needs, we continue to maintain a high standard of care and trust with our patients, while developing ongoing relationships with the people of our community.

JOB DUTIES

Perform intake evaluations/assessments, daily patient documentation, progress notes and discharge summaries.

Accurately report billing documentation, ICD.10 codes, consistent with the requirements of the APTA, specific payors or network contracts.

Develop specific therapeutic exercises and a home program to meet short term and long term/outcome goals of each patient.

Meet expected productivity guidelines as outlined in the productivity contract.

Initiate and complete required continued education requirements as outlined in the guidelines of the California APTA.

Treat each patient as a unique individual providing appropriate care, compassion and dignity.
