ABOUT OUR ORGANIZATION
We are the Heinzerling Community, and we provide compassionate, respectful care to children and adults with developmental disabilities. We offer a wide range of services in a residential setting to support and care for children and adults with severe or profound developmental disabilities. We work to create a nurturing environment in which every resident has opportunities to learn, to have fun, to feel safe, and to make choices about what happens in their lives. Our dedicated staff is well-trained to provide the expertise, assistance, and support needed to help residents enjoy experiences where they live and in the community.
The Director of Social Services is responsible for maintaining contact with parents or guardians and referring agencies for the admission and discharge process. This position provides significant assistance to parents/guardians and staff in completing and processing the required documents involved with Medicaid and Social Security, and in establishing the level of care required for all new residents. This position also serves as the residents' primary advocate and ensures that their resident rights are followed at all times.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Performs regular outreach to community services and agencies to inform regarding Heinzerling's mission, resources, and supports
- Keeps Administrator apprised of census and efforts to fill vacancies
- Serves as liaison between Heinzerling and parent/guardian, protective services, case managers, etc.
- Communicates and visits with prospective residents and families to assess needs of individuals and makes initial determination of appropriateness of placement in facility
- Secures prospective residents' information such as history and physical, psychological, educational, behavioral, and therapy assessments; communicates this information to appropriate team members during admission process
- Schedules and chairs admissions meetings for new residents; arranges visitation for prospective residents.
- Follows admission requirements of county and state regulations; ie pre-admission options counseling
- Provides assistance to family members with Medicaid, Medicare, Social Security, Insurance, and probate court issues and paperwork
- Obtains required consents from parents/guardians upon admission and annually thereafter
- Assists family members to understand resident care and programs through individual meetings, phone calls, team meetings as needed
- Prepares initial and annual social histories for all residents
- In conjunction with Program Director and Director of Nursing, notifies parents, appropriate staff, and appropriate agencies of major unusual incidents, medical emergencies and hospitalizations
- Prepares annual social service goals for IHP's, writes progress notes, and completes reviews of goals as required
- Attends individual habilitation team meetings for residents
- With team's input, reviews appropriateness of placement and transition plan for each resident on an annual basis
- Oversees spending of funds in residents' accounts for individual purchases; gives preliminary approvals for purchases, collects receipts, and ensures that purchased items are inventoried, labeled, and distributed to residents
- Serves as evaluator for facility with Ohio Department of Disabilities to submit required electronic data to establish level of care and redeterminations of level of care on the timelines established by DODD
- Stays current with Medicaid and Licensure regulations that impact the social services area and resident families and care
- Conducts annual residents' rights in-services for all staff and presents relevant information to new staff in general orientation
- Requires a Bachelor's degree (B. A.) in social work from four-year college or university, and three to five years related experience and/or training.
- Valid Social Worker license.
- Experience with individuals with moderate, severe, profound intellectual and developmental disabilities strongly preferred.
- Must pass the DODD test to be an evaluator for level of care submissions.
