Assist in facilitating smooth hospital discharges and reducing readmissions.Clinical skill set required:
Complex Wound care including Doppler compression
Simple wound care
End of life care
Medication administration
Catheterisation male female suprapubic
Bowel care
Syringe driver
Insulin administration
Doppler assessment
Compression bandagingEssential Qualifications:

NMC registration (valid and active).
A minimum of 6 months of NHS experience as a Community Nurse.
