Job Description: MAP - Social Worker

Overview

We are seeking a compassionate and dedicated MAP (Multi-Agency Partnership) Social Worker to join our diverse team. The successful candidate will work collaboratively with various agencies to support individuals and families, particularly those dealing with unauthorised issues, ensuring their well-being, safety, and access to necessary services. Responsibilities

Assess and support individuals and families facing unauthorised challenges, providing tailored intervention plans.
Collaborate with multi-agency partners, including health, education, and law enforcement, to coordinate comprehensive care and services.
Advocate for clients rights and needs, promoting inclusion and equal opportunities.
Maintain thorough and confidential case records in line with legal and ethical standards.
Participate in multi-disciplinary meetings, contributing professional insights to improve client outcomes.
Deliver culturally sensitive services that recognise and respect diverse backgrounds and experiences.
Stay informed about the latest social work practices, policies, and legislation related to unauthorised issues.

Qualifications

Bachelor's or Master's degree in Social Work or a related field.
Valid social work license or certification as required by jurisdiction.
Experience working with multi-agency partnerships and understanding of unauthorised social matters preferred.
Strong interpersonal and communication skills, with the ability to engage diverse populations effectively.
Commitment to principles of equality, diversity, and inclusion in practice.
Ability to manage complex cases with professionalism, empathy, and cultural competence.
Proficiency in documentation and case management software.

Day-to-Day Activities

Conduct home visits and client interviews to assess needs and circumstances.
Liaise with partner agencies to arrange support services such as housing, healthcare, and legal advice.
Develop and review individual care plans, updating as necessary to reflect client progress.
Facilitate group sessions or workshops addressing unauthorised issues and related topics.
Attend regular supervision and professional development sessions to enhance skills.
