We are working with a public sector client in Liverpool to recruit an Outreach Family Support Worker within Children & Young People's Services. This role focuses on delivering targeted Early Help support to children, young people and families across the city.
You'll hold a caseload of Level 3 Early Help cases, delivering home based and community support using a whole family approach. You'll coordinate multi agency plans, complete assessments, and work closely with families to reduce risk, improve outcomes and prevent escalation to statutory services.
Key responsibilities within the role:
Manage complex Early Help cases stepping down from Children's Social Care.
Complete Early Help Assessments and lead Team Around the Family (TAF) processes.
Provide targeted, intensive support to vulnerable children, young people and families.
Use Signs of Safety and Family Outcomes Star tools to measure progress.
Maintain accurate electronic case records, plans and chronologies.
Promote safeguarding, equality and anti-discriminatory practice in all work.Key requirements for the role:
Experience supporting children, young people and families in an Early Help or similar setting.
Strong assessment, planning and multi agency coordination skills.
Knowledge of Early Help frameworks, safeguarding legislation and risk assessment.
Ability to manage challenging behaviour using de-escalation strategies.
Confident working independently in homes and community settings.
Excellent communication, organisational and record keeping skills.
Contract Length: 3 months initially with the view of an extension.
Pay Rate: 19.40
Working pattern - 37 hours per week
