Our client Caerphilly county borough Council is looking for an Adults social worker to join their Physical disability & Sensory impairment team.
KEY RESULT AREAS
Practice in accordance with National Occupational Standards for Social Work and Code of
Practice for Social Care Workers.
Assessment and reviews of service users recorded outcomes, risks and needs.
Assessment and Care Management.
Multidisciplinary Working.
Service Development.
Demonstrate professional competence in social work practice.
Manage and be accountable, with supervision and support, for your own social work practice.
Ensure integration of service provision with Health Trust and Social Services through regular
liaison with staff in related organisations during daytime, evenings and at weekends.
