Job Overview
We are seeking an experienced Reablement Occupational Therapist to join Adult Social Care services in Enfield. The postholder will deliver a high-quality, strengths-based and person-centred occupational therapy service within a reablement setting, supporting adults to regain or maintain independence following illness, injury, hospital discharge, or functional decline.
You will work across community settings, completing functional assessments, developing outcome-focused reablement plans, and enabling service users to achieve meaningful independence in daily living activities.
Key Responsibilities
Carry out occupational therapy assessments in home and community environments
Deliver strengths-based, outcome-focused reablement interventions
Support adults with personal care, mobility, transfers, domestic tasks, and community access
Assess functional ability, risks, and environmental barriers using clinical reasoning
Recommend and arrange equipment, assistive technology, and minor adaptations
Identify need for major adaptations and make appropriate referrals
Provide moving and handling advice, risk reduction strategies, and safe practice guidance
Work collaboratively with reablement staff, social workers, health professionals, and partner agencies
Support timely hospital discharge and admission avoidance
Monitor progress, review outcomes, and adapt intervention plans accordingly
Complete Care Act assessments, mental capacity work, and safeguarding duties
Maintain accurate, timely and compliant case records
Contribute to supervision, training, and service development
Manage a varied caseload, including complex and urgent cases
Person Specification
Essential:
HCPC registered Occupational Therapist
Experience working within adult social care, reablement, or community OT settings
Strong knowledge of the Care Act 2014 and Mental Capacity Act 2005
Experience in equipment provision and moving & handling assessments
Ability to work independently and manage a community caseload
Strong communication, risk assessment and problem-solving skills
Ability to work collaboratively within multi-disciplinary teams
Commitment to strengths-based, person-centred practice

Knowledge of hospital discharge pathways
Experience supporting complex adults with multiple needs
