Community Occupational Therapist
up to £40.00 per hour | Greenwich
Overview
I am currently recruiting for an Occupational Therapist to work with my client in Greenwich, London.
Main Duties
Undertake assessments with service users, families and carers to identify needs and promote independence, safety and wellbeing.
Work collaboratively with multidisciplinary teams, partner agencies and external organisations to develop and deliver effective support plans.
Assess and recommend specialist equipment, adaptations and environmental modifications to support individuals in their daily lives.
Develop, implement and review rehabilitation plans, care plans and support strategies tailored to individual needs.
Manage a caseload, monitoring progress and ensuring interventions remain appropriate and effective.
Coordinate assessments and produce high-quality reports to support decision-making and service provision.
Present recommendations and assessment outcomes to relevant multi-agency panels and meetings.
Build positive working relationships with service users, carers and professionals to achieve the best possible outcomes.The contract is 35 hours per week.
Requirements
At least 2 years postgraduate experience
HCPC registered
Bachelor's degree or higher in Occupational Therapy recognised by relevant professional bodies.
Clear DBS (we can put this in process for you)What's on offer
